Today, I'm sharing a link to a Boston University Medical School paper that was focused on reducing readmissions and establishing a transition program that works. It uses a nurse Discharge Advocate to deliver patient education and take the patient through the After Hospital Care Plan. In addition to having a tradition "live" nurse, patients also have the option of selecting a computerized avatar Discharge Advocate. They have found that this second option:
- is particularly helpful to patients with low health literacy levels
- takes 51 minutes vs. 81 minutes for a live Discharge Advocate
- is preferred by 74% of patients
- encourages patients to be an active participant
